After filling out this form I will review it and respond if the project has been approved. If approved I will contact you about setting up a consultation, at the consultation your appointment/s will be made. Email responses may be found in your SPAM folder.

- Free range abstract tattoos will take first priority.

- If you do not get a response back from an inquiry about your tattoo, it's not because I don't like your idea, it is because I do not feel confident I can provide you with my best work with the subject or idea/s given to me. If you have sent an email in response to a current conversation and have not received a response in one weeks time, please check to see if your email actually sent and or check your spam folder. Response time is normally 4-5 days but may vary due to holidays, weekends, etc. Once an email has been sent please reply to that email only. In order to keep things organized. Do not send a new email each time.

- Non Refundable $100 deposit is required to schedule an appointment.

-Cancellation \ Rescheduling Policy

--Late arrival: Please be on time, not early for your appointment. Door will be locked. Please knock. There will be a 30min window for late arrival. Any time after that your appointment will be rescheduled and your deposit for that day will be voided. --Confirmation: No response to your confirmation email will result in cancelation of your tattoo appointment. A new deposit will be required in order to schedule an appointment. --Design change: Approval from the artist must be given in order to change your tattoo design. If your idea changes for your tattoo without reasonable time for design change, and or for a request that is above the artist skill level your appointment will be rescheduled. A new deposit will be required as well. --Rescheduling: If you need to reschedule your appointment one full week notice is required. Any rescheduling after that the deposit will be non-transferable and non-refundable. A new deposit will be required to make the new appointment. --Deposits made for multiple appointments are for the scheduled day only. The use of multiple deposits for one session is not permitted.
